Candidates must fill out the Standard Form of Assam Gazette (Part IX).
Attach self-attested copies of all relevant documents and a passport-size photo (signed by the candidate).
Staple the photo to the top-right corner of the form.

Submit the application within 15 days from the date of publication either:
